Privacy Policy
This privacy policy (hereinafter "Privacy Policy") describes how and what personal data University of Oklahoma Stephenson Cancer Center ("We, we, us, our or ours") collect and use, as well as your rights in relation to that personal data, when you visit or access the Insight application on your mobile device.
This Privacy Policy does not cover any information that may be collected about individual research participant ("You, your or yours") through sources other than your use of Insight application. Insight may link to other websites or mobile applications, but this Privacy Policy does not apply to any of those linked websites or applications. The information in this Privacy Policy applies only to the University of Oklahoma's use of your personal information as it may be received from your access and use of the Insight application. To the extent allowed by applicable laws and/or regulations, we will take all reasonable measures to protect the confidentiality of your personal information and your identity.
Insight application is developed to assist your researchers to conduct scheduled or random surveys. We utilizes third party providers' cloud servers to store your personal data. Your personal data collected, resulting from your use of the Insight application, will be stored by us for the duration of the research you are part of.
By using the Insight application on your mobile device you consent to our collection, use and storage of your personal data as described in this privacy Policy. Your personal data (or that of your child if you have previously provided consent on their behalf) may include but it is not limited to your personal data, such as your name, sex, age, date of birth, Social Security number, contact information, body weight, height, race, ethnicity, family history, medical records, answers to questions and call logs using Insight.
You can withdraw your consent for future collection, use, and/or disclosure of your personal data at any time. If you want to make this request, you need to contact (405) 271-3150. Withdraw of your consent may affect your participation in the study.
We may share your personal information only in a lawful, fair, and transparent way in relation to you.
We will be sharing your personal information with your researchers in compliance with applicable privacy laws. Your personal data will generally be kept no longer than needed for the purpose we collected it for or as required by applicable law.
To the extent applicable some or all of the following rights may apply to you:
- The right to request your personal information collected to be correct
- The right to withdraw your consent for the use of your personal information at any time
- The right, in some circumstances, to receive your personal information in a structured, commonly used and machine-readable format and the right to provide your information to a third party
- The right to strict confidentiality of your personal data when it is used/shared
- The right to limit the use/sharing of your personal information in certain circumstances
- The right under some circumstances to request the erasure of your personal data
- The right to file a complaint with a privacy protection regulator if you believe any of the rights above have been violated
